Status Reports
The Foundation requires a status report for all approved projects on a quarterly basis until the project has been completed. The report should summarize the progression of work proposed in the grant application such as key accomplishments, challenges and/or delays, results and impacts.
Quarterly Deadlines
- First Quarter (January – March 31)
- Second Quarter (April 1 – June 30)
- Third Quarter (July 1 – September 30)
- Fourth Quarter (October 1 – December 31)
A final status report on how the work progressed and documenting the final overall cost of the project and use of funds received is due upon completion of the project.
Work Completion Certification
Grant disbursement requests are to be submitted by completing the Work Completion Certification Form.
The due date for submission of work completion forms is by the close of business on the 20th of each month to ensure payment by the end of that month.
